Under the terms of the GSK-Futura agreement, GSK will pay 65 per cent of the clinical development program costs for the drug and Futura will pay the remaining 35 per cent.
The co-development program, to be run and managed primarily by GSK, is expected to comprise up to three studies involving approximately 1,500 men with ED and several additional safety studies to support the existing studies already completed by Futura. The drug is also likely to be human-tested in India as part of the global trials.

Why did the drug work so effectively with some and not others? The basic analysis to date suggests that people who suffer most from a fall in their performance at high altitude are the most likely to benefit from sildenafil citrate. Neither the participants nor the researchers knew whether the trial included a placebo or one of the two sildenafil doses, 50 mg or 100 mg. The high altitude simulation was achieved by changing the mix of air. The cyclists began breathing the high altitude mix starting one hour before the exercise session and continuing through the session. The simulation did not include the lower air pressure that would occur at altitude.

Researchers analyzed the changes in each individual's performance under various exercise conditions and also compared the group's performance under different drug conditions. Responders versus non-respondersFour of the 10 participants responded to sildenafil citrate while the remaining six did not, Friedlander said. The responders showed the greatest drops in stroke volume, cardiac output, and cycling performance between the sea level and high altitude trials without the drug. "Without sildenafil, their performance went down more than others," Friedlander said. "With it, it brought them back up to the levels of the non-responders."

The results suggest that the responders experienced a greater degree of constriction of the vessels in the lungs at altitude and therefore benefited more from the vessel relaxation effects of sildenafil citrate.

An outcome of the research work is the realisation that sildenafil citrate could be considered as a treatment for those who suffer most at altitude but, because of side effects that can include severe headaches and the apparent inability to help some people, it should not be taken as an exercise aid by everyone.

The bigger picture:
This study adds to the scientific knowledge of what physiological factors limit performance at altitude, including the role that cardiac output plays. For instance, physiologists don't know why some people have trouble at altitude and may develop illnesses such as acute mountain sickness or high altitude pulmonary edema while others adapt quickly.

Studies like this may help identify some of the underlying differences between people and lead to better treatments. In future studies, the researchers want to identify: * what steps individuals could take to acclimatize before they go to altitude* who is likely to acclimatize quickly at altitude and who may need additional help* how to minimize performance declines at altitude. Generic drugs busting blockbuster profits

Blockbuster drugs have earned billions in profits for big pharma manufacturers, but patents are expiring on some of the most popular products, opening their formulas to low cost generic copies and threatening future profits for drug companies desperate to produce the next generation of stars for our medicine cabinets.

This week's announcement by Merck & Co. Inc. that it will lay off 7,000 employees, a 10th of its work force, and shutter five of 31 manufacturing sites to save $4 billion through 2010. The company said it would also look to acquire another company to build its product pipeline. Other pharmaceutical companies, including Pfizer Inc. and Wyeth, have also cut jobs and closed plants during the past year to head off expected declines in profits.

The raw material production of drugs are very cheap, as proven by the generic copies, but the big overheads - mostly in marketing are very expensive. Big pharma has long claimed that the cost is in research, but the facts are the biggest cost is in top heavy administration overheads, heavily protected by tarrif walls and patents. Many of the big pharma companies produce their biggest selling drugs in low cost countries like India. However, they started to cutail that activity when they found it increased the risk of generic alternatives being produced.

In India, you could break a patent by producing the same formula with a different production process.

"They've had this productivity problem now since the late 1990s," said Christopher-Paul Milne, assistant director of the Tufts Center for the Study of Drug Development in Boston.
"I think they were hoping it would just go away if they kept doing things the way they had been doing them," Milne said. "But for better or for worse, [they're realizing that] the blockbuster model really is not going to be sustainable."

It does take years to develop a new drug after securing approval from regulators. And there's no telling which will be hits.

Many of the big Pharma companies are branching out to overcome the the uncertainties. The issue of generic production, coupled with other industry issues, has caused some pharmaceutical giants to begin branching out.

Over the past few years, many of the big pharmaceutical companies have set up partnerships with biotechnology businesses that make niche drugs, theorizing that money can be made by offering many small products rather than a few major ones.

Others have looked to new scientific methods that could be used to resurrect failed products.EG: Pfizer Inc., is now working with a Gaithersburg company, Gene Logic Inc., that says it might be able to find new uses for some of Pfizer's stalled drugs.

The impetus for Merck's announcement Monday appeared to be the 2006 patent expiration of Zocor, a cholesterol medication that was the second-biggest seller in the world last year with $5.9 billion in sales, and problems with the painkiller Vioxx. Merck had looked to that drug as its next potential gold mine, but instead had to pull it from shelves last year amid accusations Vioxx increased the risk of heart attacks and strokes.

"The actions we are announcing today are an important first step in positioning Merck to meet the challenges the company faces now and in the future," Richard T. Clark, chief executive officer and president of Merck & Co. Inc., said in a prepared statement.

A generic version of Zocor could cause a dip in sales not only for Merck, but for its cholesterol competitors such as Lipitor, a Pfizer drug that had worldwide sales of about $12 billion in 2004. That category of drugs was the biggest seller worldwide last year, combining for about $30 billion of the industry's more than $500 billion in annual sales.

Other blockbusters, according to industry consultants NDCHealth of Atlanta, include AstraZeneca PLC's Nexium, the heartburn medication dubbed the "purple pill" that recorded $3.6 billion in U.S. sales last year, and Pfizer's depression medication Zoloft, with sales of $3 billion.

Sales of Viagra have been declining because of "competition," Pfizer said. The drug garnered $1.68 billion last year, down 11 percent from 2003 sales.

"The industry itself is under a lot of pressure; there will be some changes," said Frank Palumbo, director of the University of Maryland's Center on Drugs and Public Policy. "I still think the salvation of a large company is with their blockbuster drugs and they will continue to look for those. Big pharma cannot sustain itself without being able to sell high-profit drugs at high volume."

But industry analysts expect that sales of blockbuster drugs will continue to decline as patents expire. More than $80 billion worth of such drugs face patent expiration by 2008, according to a report by industry researcher Cutting Edge Info, based in North Carolina. The report says brand-name drugs typically lose up to 30 percent of their market share as soon as the first generic version arrives.

Once multiple generics are on the market, the brand-name drugs lose up to 90 percent of market share. Without backup blockbusters, the pharmaceutical industry will likely lose revenue unless it makes some changes, experts said.

"I think that message is beginning to resonate," Tufts' Milne said.
"The paradigm is changing in that they are reaching out to partnerships with more players. ... They should be doing that, and some of them are moving in that direction, but again, the question is: Is it going to be fast enough," he said.

Doctors are prescribing Viagra to a small test group of children with a fatal, lung-heart condition. The drug, normally prescribed to men for middle-aged male impotency, helps children with pulmonary arterial hypertension to breathe more easily by expanding blood vessels in the heart and lungs.

Evidently, some 4,000 Britons suffer from the incurable condition, including 320 children, and doctors have started controlled trials of the drug.

In a year-long Canadian study, Viagra was given four times a day to 14 children with the condition. The average dose was 40mg, double that taken by impotent men.

Before treatment, the children could walk an average of 324 metres in six minutes. After one year of treatment, they could walk a further 154 metres in the same time and blood flow increased by 20 per cent. Patients were given a 37 per cent chance of surviving a year, but at the end of the trial all 14 were still alive.

Iain Armstrong, the chairman for the Pulmonary Hypertension Association, said: "Physicians discovered that the receptors in the lungs are similar to the receptors in the penis. So they thought if it [Viagra] can work for erectile dysfunction it should work for pulmonary hypertension. The results are encouraging."

Dr Joanna Pepke-Zaba, the director of the Pulmonary Vascular Disease Unit at Papworth Hospital in Cambridgeshire, said: "We have tracked adverse reactions such as the possibility of boys having erections and being put in an embarrassing situation. It has only happened once in a boy". However, she went on to say there was no hard evidence to suggest it was due to the Viagra.

Fran Georgel, 43, from Buckingham, Kent, whose nine-year-old son Robbie has been taking a drug similar to Viagra for five years, said "Once Viagra is confirmed safe, it should definitely be licensed."

Pfizer, the manufacturer of Viagra, intends to seek a licence for the drug as a treatment for pulmonary hypertension when the trials are complete.
